Weeks after a massive warehouse fire sent thick smoke across Boyle Heights and neighboring communities, many residents are still asking the same question: What happens after the smoke clears?

While scientists cannot yet say whether the Boyle Heights fire will lead to long-term health problems, a newly published study offers important clues about one chronic condition that may be affected by prolonged exposure to fine-particle air pollution.

Researchers found that people living with rheumatoid arthritis who were exposed to higher levels of PM2.5, tiny airborne particles produced by combustion and smoke, experienced more disease flare-ups and higher levels of inflammation than those exposed to lower levels. The findings were published in the peer-reviewed Annals of the Rheumatic Diseases.

The study does not examine the Boyle Heights fire or wildfire smoke specifically. Instead, it adds to a growing body of evidence suggesting that long-term exposure to fine-particle pollution may make certain chronic inflammatory diseases more difficult to manage.

Why this matters after the Boyle Heights fire

Scientists are increasingly investigating that possibility.

What is PM2.5?

PM2.5 refers to microscopic particles measuring 2.5 micrometers or smaller.

Because they are so small, these particles can travel deep into the lungs and even enter the bloodstream, where researchers believe they may trigger inflammation throughout the body.

PM2.5 is produced by:

  • Smoke from fires
  • Diesel trucks
  • Vehicle traffic
  • Industrial facilities
  • Power plants
  • Wood burning

Southern California residents are exposed to PM2.5 from multiple sources throughout the year, including wildfire smoke, freeway traffic, ports, warehouses and industrial activity.

What the new study found

Researchers followed 1,070 people with rheumatoid arthritis over four years, analyzing more than 12,500 outpatient visits while comparing disease activity with local air pollution levels.
Among six major air pollutants studied, PM2.5 consistently showed the strongest association with worsening disease.

Patients exposed to higher levels of PM2.5 experienced:

  • More rheumatoid arthritis flare-ups
  • Higher overall disease activity
  • More swollen joints
  • More tender joints
  • Higher inflammatory scores used by physicians to monitor disease progression.
    Perhaps the most notable finding was that longer exposure mattered more than a single bad air-quality day.

Researchers found that flare risk increased after roughly two weeks of elevated PM2.5 exposure rather than immediately following short-term pollution spikes.

The study answers one important question, but it does not answer all of them.

Although this research was conducted overseas, California has become increasingly familiar with prolonged periods of unhealthy air caused by wildfire smoke, industrial emissions and heavy transportation corridors.

Communities near freeways, ports and industrial facilities, including parts of Boyle Heights, Southeast Los Angeles and the Inland Empire, often experience higher levels of PM2.5 than cleaner suburban areas.

For people already living with chronic inflammatory diseases such as rheumatoid arthritis, maintaining good air quality may be more important than previously understood.

The researchers concluded that physicians should encourage patients with rheumatoid arthritis to reduce exposure during periods of poor air quality while policymakers continue efforts to improve air quality overall.

What people with rheumatoid arthritis can do

Health experts generally recommend that people with chronic respiratory or inflammatory conditions pay close attention to local air-quality alerts.

On days when PM2.5 levels are elevated, patients may consider:

  • Limiting prolonged outdoor activity.
  • Keeping windows closed when smoke levels are high.
  • Using high-efficiency indoor air filtration when available.
  • Following treatment plans prescribed by their rheumatologist.
  • Discussing worsening symptoms with their healthcare provider.

These recommendations are not specific to the new study but reflect common public health guidance for reducing exposure to fine-particle pollution.

The Boyle Heights fire raised urgent questions about what residents breathed during the days smoke blanketed nearby neighborhoods.

This new research does not prove that the fire will cause rheumatoid arthritis flare-ups or other chronic illnesses.

It does, however, provide another piece of evidence that prolonged exposure to PM2.5 may have health effects extending beyond the lungs.

As researchers continue studying the long-term consequences of smoke exposure, the findings reinforce a broader public health message: cleaner air is not only about preventing respiratory illness. For many people living with chronic inflammatory diseases, it may also help reduce painful flare-ups and improve quality of life.
